I’ve got a situation. To make the toilet fit with outflow - got a short 90 from vertical to horizontal out of toilet followed by a 2 foot horizontal to a short 90 connected to a elbow 90 back to horizontal. Got some gurgling and regular incomplete flushes. The short 90 connected to the elbow 90 are accessible. Outside of demoing from toilet to connected 90s, anyway to fix this?

Regular 90s are simply not permitted in the horizontal plane. On the first floor of any building, a cleanout is needed for each 135 degree of horizontal bends.
There is also a vent required within 5' of the closet bend.

There should be a vent on this somewhere or the 90's should be changed. Too much water flowing too quickly which is displacing too much air too quickly which is making for a irregular flushes. There's not much advice to give other than change the piping. But change it to what? We don't know what's possible you haven't provided enough info to give a good plan.

I think i would have tried to 45 out of there and then into a tee, but I cant see it that well.

Your photos don't clarify things. I suspect UP is on the LEFT in the first photo. I don't know about the second.

I’m fine tearing it out and rebuilding. Question would be with the 90 right at the joist, there isn’t much pipe left if I cut that off. Good grief!

Two 45s instead of the 2 90s might be an improvement.

I'm not a plumber so only a guess.
