Need help identify bathtub faucet brand/model

Hello,

I'm trying to fix our bathtub faucet but can't find the brand name anywhere. I've checked repeatedly with no luck. Could anyone identify this model for me? It has the words "OFF," "COLD," "WARM," and "HOT" written in a fancy-style font (see photo below). The white part linking the step to the handle is damaged, and I want to replace just this piece instead of the entire unit. Thanks for any help!
