Missing venting in bathroom (likely whole house)

Moved into this house in 2018 and there have been no shortage of plumbing related problems. The latest was a leaking toilet, which I think was really a slow to drain toilet based on the complete lack of venting. I cut a hole in the floor to see the extent of the leak and found the following layout (all horizontal, no slope).

Besides the missing slope, I have the following questions;
1. Should any of the Wye fittings be turned up 45deg, particularly the one on the 3” line?
2. Would it be acceptable to have a wet vent off the shower for the toilet?
3. Would the sinks need additional venting if a wet vent was added above?

Side note, I think the only vent in the system is on the main line in the basement.

Any comment/suggestions appreciated.
