3-4 inches 45 degree elbow to reroute existing building sewer

Good day Eeveryone,

There is an existing building sewer that all house utilities drain into, as shown in green. We need to reroute this line since the existing sewer main at the rear of the house will be abandoned, and we plan to connect the building sewer to the new sewer main at the front of the house. This existing building sewer is located below the slab ground, making it very difficult to overhaul the entire plumbing layout.

My question is: would it be acceptable to install three 4-inch, 45-degree PVC elbows with a double cleanout afterward? The elbows will be located under a thick pavement, which limits us to placing only a double cleanout outside the house. We are located in the Commonwealth of Northern Mariana Islands, but I haven't found any applicable local plumbing codes.

If this is not a suitable option, I’m open to any suggestions.
