Need some help in basement washroom and plumbing

I am from Kitchener, ON. I have done this plumbing for my washroom in basement recently and the inspection failed it and did not give any information why its not failed and wanted me to go with licensed plumber. Need some help to understand what did I do wrong and how to fix this. Thank you.
