Toilet tank sludge and water spraying

2 toilet tanks in my home have this thick nasty sludge (see photos). When you flush, the hose in the tank will come loose (like the pressure is too high?) and spray water everywhere. I had been experiencing issues with very low water pressure for about a week now in the showers if 2 are showering at the same time which hasn't happened before. My master bathroom toilet is working but the water won't shut off... like the pressure is high at that toilet, too. The rest of my water lines seem fine. I do have a water softener if that makes any difference.
