Kohler K-4609 Pinior 1.6 gpf circa late 90's or early 00's

Any idea what this toilet is and the rebuilt kit? Our plumber repaired a few months ago, but it's started running again.

You don't need anything specific about that model to deal with "running". Call the plumber back. What repair did they do?
That flapper should have been replaced.
Use leak disclosing tablet from hardware store or drops of food coloring to see if it is the "flapper" leaking into the tank.
