It's usually that the flapper has worn out and the water is running out of the tank slowly. Try turning off the water to the tank, mark the level that it's at with a pencil, and come back in an hour. If it has fallen, then water is running out of it to the bowl.
The rest of this advice is based on the assumption that your tank inside says ST743S#01. Does it? If so, then I know which toilet you have and thus can point you to a particular flapper. If not, then tell me what it says starting either with ST or MS.

Should take about a minute. Definitely an easy DIY job.
If you cant find the Korky 2021, you can use the Korky adjustable 3" flapper, Model 3060. http://korky.com/Flapper3060BP.html You would then spin that little white band around to "set" it to close at the right time on your toilet.
