Need to replace toilet with wide base and big trapway

I have a relative that's disabled....a big guy that's disabled physically and mentally. His toilet clogged often so we replaced it with a Champion 4 that has the 2 3/8 trapway. That seems to have fixed the clogging problem for the most part. But now we've found he rocks the toilet so hard to get up that the toilet bolts aren't securing it to the floor. We've had that fixed a couple of times but he eventually pulls it loose again. We've already had grab and support bars placed around the toilet for some time - may be helping but clearly not enough.

I think there are some Toto toilets that have wider bases, maybe there are others. But the Toto's I've looked at don't have the big trapway. I do NOT want problems with clogs so Im afraid to go with a smaller one.

Anyone have ideas where I could look to find a decent toilet that's tall or ADA compliant, elongated, wide base and a large trapway?
 
American Standard Vormax. Best gravity toilet on the market I've seen. Will flush a Honda Civic.

New flange and bolts. Use 100% GE silicone clear non paintable caulk all around the base. Leave say 4 inches open in the rear for possible leaks. That will seal the bowl to the floor. Make sure you angle the caulking gun towards the bowl to get the silicone under the bowl. Let cure for 24 hours before using the toilet.
