Help Identifying Shower Model

I am hoping for help locating a suitable replacement shower handle/trim and cartridge for my existing shower -- picture below.
My shower started dripping/leaking after a gazillion years of excellent performance. My plumber let me know we may be able to replace just the cartridge to resolve the drip/leak.
However, when he went to remove the handle, the screw was completely stripped. We cannot remove the handle without cutting it off and so taking the shower out of service.

I am hoping perhaps someone might recognize this old model, and maybe direct me to a compatible replacement?
For more detail, my shower handle points up for off, and only turns left for on, 1/4 turn is fully on maximum hot. I am not sure when it was installed, but I expect it may have been more than 20 years ago.
