Frequently clogged of toilet

I have a new issue in my house where the toilets are starting to get clogged up every week. They unclog fine, but I don’t think it’s normal that they get clogged up every week. I have also snaked them. My house was built in 2000 and I read that this may be because it is an old toilet, but I can’t find the date on the toilet.

They are a Gerber, 1.4 and 1.6 stamp on the lid. The number on the back of the tank is 821-21-399 (which I can’t find in a Google search). Can anyone give me advice what to do? Is the toilet of an age where I need to replace it?

If you have a toilet that clogs every week, I would replace it regardless of age.
 
With the passage of time, minerals build up in the internal trapway of all toilets, even the ones with "special" coatings (Toto and and Am. Std have on some of their models), but I've found in real life, build up still happens. It causes poor flush performance.
Snaking will NOT clear build up, so the ultimate solution, if you are sure you don't have an actual drain issue in the pipes is to replace.
Single ply toilet paper made for 'delicate' systems might keep you going longer between clogging.
