Cleanout Placement clarification

Hi all,

I am designing my DWV for basement bathroom and bar sink/powder room. I'm hoping to apply for permits within the next 2 weeks. We are under 2021 UPC and 2021 IBC.

In the bar sink and powder room area, I am trying to figure out how to place my upper terminal cleanout to keep a clean look. I am hoping for some cleanout placement clarification. 707.4 states "Cleanouts shall be permitted to be omitted on a horizontal drain line less that 5 feet in length unless such line is serving sinks or urinals." 707.9 states "Each cleanout in piping 2 inches (50 mm) or less in size shall be so installed that there is a clearance of not less than 12 inches (457 mm) in front of the cleanout."

I am attaching a rough draft of my drain layout. Red wall is load bearing.

1) Is a lavatory considered a "sink" for this code? Based on Figure 707.4, it appears since my lavatory line is less than 5', it does not require a cleanout?
2) Can my upper terminal cleanout be in the cabinet under my bar sink? (I'm assuming because it is a bar "sink" and not a lavatory, a cleanout is required.)
3) Can I then add a cleanout for the toilet where I vent it? I would prefer this cleanout be in the wall, opening into the mechanical area. Would it be better to have it as a cleanout in the floor instead of the wall? I have space for either.

I am open to other plumbing layouts, I have just been messing with it for too long and wanted to get some feedback before I continue on with my design.

2.yes. Inside cabinet is fine
3.yes, c.o. In wall on back side is fine. Clean out/test tee or wye fitting. Don’t use conventional tees for clean outs

When water Jetting a kitchen waste line in a multi story building I like to install a "conventional Tee"

The reason being I can jet upstream as well as down stream at the base of the stack or on the accessible horizontal
