Clarify DWV best practices

Need to correct some issues in the DWV under a couple washrooms (backwards slope, hard 90s, no cleanouts), and looking to confirm what would be best practice for a couple points:

Options #1-2 are to jog from between the joists to turn across below the joists.
-is either of these better than the other?
-is a cleanout even worthwhile here if this gets enclosed in the ceiling? Could potentially add an access panel, but this is above the garage

Options #3-4: reconfugure shower 2" trap arm (replace existing 90 with 1/4 bend + wye)
- are either of these acceptable or good practice? Connecting with a wye 3x3x2 wye further downstream is difficult due to space constraints.

I think number 2 looks better . number 4 has a clean out 3 doesent. clean out might be more desireable
