It seems like someone is leaving a faucet open while shutting a diverter possibly or there is a point of use water filter attached to a faucet and the operator is shutting the filter off and leaving the faucet handles in the open position. There are a few other scenarios as well that could cause hot/cold mixing. e-plumber

I doubt a stem would correct this problem, if it did, you would need to replace every stem with the super duper type.
