You may want to call a pro for this one, they might have some ways to make a proper repair. You are on the right track as far as what you need to end up with. You might consider breaking out the concrete around the pipe down far enough to splice in new PVC, but that could cost you as much as a professional plumber once you bought or rented the tools to break out the concrete, make the repair then patch the concrete.
