The threads are for clean-out plugs only.
The correct way to do the install would be to melt the complete clean-out fitting out of the cast iron hub, remove the fitting.
Install another clean-out fitting, then make your branch connection.
If the existing clean-out is on the vertical, it can be a sanitary TY.
If horizontal, it needs to be a Y.
