Repair or replace bath tub stopper

I have a bath tub with a lever on the over-flow plate that operates the stopper. The lever now feels loose and now will not operate the stopper.

Any idea what the problem may be?
Any suggestions or pictures on how to repair or replace it? Thanks. Jeff
