Radiator hook up

Where does the hot water from the boiler hook up to a radiator ?? top or bottom. Does it matter ?? Whger does the valve go ?? input or out put ? Some of my radiator have 3 pipes connected to them. they do not get as hot as others. Could this be the prolbem ?
