Floor flange

The floor flange on my toilet came loose today when I was attempting to install new closet bolts. I need to reinstall it or install a replacement flange, but I cannot see how the original was initially installed. The flange and the soil pipe are both made from cast iron, but they don't appear to have been attached to one another (the flange does not have any holes or bolts that could have been used to attach it to the soil pipe). The flange also does not appear to have been attached to the subflooring (again, it doesn't have any holes or bolts that could have been used to attach it to the subflooring). My toilet sits on a slate floor that was probably installed 30 years ago, and there is some kind of mortar or grout under the tile and surrounding the flange and soil pipe. How can I reattach this flange, or is there some other product that I should purchase and install?
