Moen makes the divertor you are looking for, it screws onto the shower arm and has two outlets, one for shower head, one for hand shower, with a push button to control flow between both shower heads.
Sorry I can not recommend any shower bars, but I imagine moen or delta manufactures them, with instructions included.
