If you have a 4" pipe coming up for the water closet you can get a flush fit floor flange and simply glue it into the pipe. If you have a 3" pipe there remove the concrete around it and glue a regular floor flange to the pipe so that the bottom of the flange sits level with the floor.
As for the tub you might have to open up the concrete a little bit more to fit a p-trap to the pipe in the floor, try to include a y and a cleanout between p-trap and waste and overflow, this will save future headaches.
