If you can see water in the drain, then I would assume that there is a p-trap installed. Does a flame flicker when placed over the drain?
Another problem could be rodent droppings in the bathroom somewhere.. I have seen situations where mice and rats got between walls and left an awful mess. All openings had to be sealed off. and the wood was painted with Zerolac to prevent the smell from coming through.
