Fireplace + Chimmey Repairs

On a two story house, the brick chimmney has started to separate from the house. At the top, it is at least 3 inches away from the house. # 1. Any estimate as to what I might expect for repairs? #2. Will a homeowners policy cover the repair? Please advise
