Toto seat loose

I have a one piece Toto toilet. The soft close seat is loose on one side. The bolts are metal. I tried turning the bolt clockwise and the bolt didn't tighten. Has the other piece underneath the porcelain likely dropped off the bolt?

I went back to try to tighten the seat again and now it is obvious the rubber piece for the toilet hold down is still there. I was persistent and was finally able to tighten the seat down.
