Cover for Basement Floor Drain

Is there a basement floor drain cover that has connections for hoses from the A/C drain, water softener drains, etc? With so many homes having this need, it would seem odd if there isn't such a thing. I had a water softener installed and the installer just shoved the hoses into the floor drain which in my part of the country is illegal--an air gap is required to prevent potential siphoning waste water back into the fresh water supply.

I've been to a number of plumbing supply houses and no one has heard of the type drain cover I'm looking for, so I decided to make my own. I want to use a plastic drain cover, but haven't been able to find one in 6-1/4".

Ever heard of a floor drain cover with connections for a water softener hose that provides an air gap? How about someplace to get a 6-1/4" PVC/plastic cover? Thanks for any help.
 
No, but that doesn't mean they don't exist. I have seen metal floor drain covers that have a funnel permanently attached and the pipefitter would run the drain lines so they ended one inch above the rim of the funnel. If the drain piping were to enter the funnel it would not have the required air gap.

You may need to fabricate what you want from stock materials. That might mean you will need to get a piece of plastic from a plastics supplier and cut it to fit your floor drain, drill the necessary holes in it to allow the water to flow through and then build the air gap assembly to which you will attach the drain tubing.

Specialty items such as that are seldom supplied in plastic because they are usually installed in commercial buildings which usually specify cast iron drains with brass covers.
