Unfortunately for you whoever installed that drop ear elbow didn't screw it into any blocking.

I'm in the middle of a bathroom remodel job and am unable to remove the shower head stem. From this picture below (taken from inside the wall) I believe it to be threaded into the 90 deg coupler, but if it is I cannot get it to budge. I've sprayed some WD-40 on it without any change. Does anyone have any suggestions on how I might remove this stem?

Unfortunately for you whoever installed that drop ear elbow didn't screw it into any blocking.
Assuming you are replacing it, just get a 10" pipe wrench. Normally, a 12" channellock would also do it. Even though the drop ear is not secured, that should turn out. The treads on the chrome arms have been known to break,and you will have to cross that bridge when you come to it.
Use a bigger wrench, or more pressure on the one you do have.
