Found this inside of a slab home today (mechanical room; house built in 2007) within about 6' of where main drain goes into slab. The soil moisture is my concern and it did not seem to change after running a lot of water in all baths. Any idea what this is?

Not a good enough picture to be sure. it is probably a cleanout cover, but it could have something to do with a back water valve. It is NOT the usual way to install a cleanout, so you would have to clean more of the dirt out and possible remove the cover to be sure which it is.

Thanks. I was thinking back water. A plumber will take a look in the next day or two and see where the water is coming from.

It's a PVC Backwater Valve, dig the dirt out of the cover and use a sitck of wood to unthread the lid, inside you will most likely see the flapper.
