Can you ,per the international plumbing code come off above the middle of your horizontal waste @ a 45 degree angle(with a wye or combo) on a floor sink?

not unless that 45 has been rolled up so that it rises vertical. You can not flat vent

The IPC allows so many things that it might also permit that.
