Toto Ultramax 12" rough-in, but sits 2" out from the wall

Terry,

I just installed an Ultramax MS854114S on a 12" rough-in. It is sitting much too far away from the wall. I found the following post on your old board - it suggest I should be measuring a 3/4" gap from the top of the tank to the wall, when in fact I see 2" (!). It just doesn't look right in the room - it sticks out way too much.

What could I have done wrong? (the toilet is sitting properly on the closet pipe and the bolts lined up with the holes in the toilet - there doesnt seem to be much that can be done adjustment-wise :(

11.25" or more will work.
At 12.0" the toilet will have 0.75" behind the toilet.
At 11.5" you will have 0.25" behind the toilet.
