Vent for Bathroom Sink

Can I have my vent come up right over the drain, make a turn then go down to the 4 inch main stack/vent? Does the vent need to be sloped up on its way to the main vent? If so, what tee do I use at the junction between drain and vertical part of vent? (sanitary tee) Also, if my sink is within 4 feet of the main do I need a vent ( I think yes ).
