Please help in identifying these old Delta fixtures

Hi:

I'm repairing a set of 6 shower control valves in a locker room shower. They're all old Delta stuff. I've successfully replaced the springs and valve seals, but I'm short some handles and cover plates.

Pictures of the handle/trim plate are here:

1. Any ideas where I can get replacement handles? (could be different style, but shaft bore needs to be the same, etc)

2. Any ideas where I can get replacement trim plates? (could be different style, but bolt pattern needs to line up, etc.

3. Any idea what the thread pitch is to hold the handles on? (need to replace a bunch of the screws that hold the handles on)

Have struck out at Home Depot... Suggested online source?
