does the flange/ring go on top of the tile or does the tile go around the flange/ring? (I really don't know the terminology but I'm not the one that's doing it, just the one doing the research!)

The flange should rest on top of the finished floor regardless of flooring material. It is also to be anchored to the sub floor. This means 4 holes need to be provided for screws. My recommendation for those screws is #12 stainless steel long enough to go through the sub floor. The flange should also have a stainless steel ring.
