Default Toilet Noise after Flush/Sudden Pipe Break

We remodeled our bathroom a few months ago, and have had problems with our toilet ever since. We re-used the toilet (nothing wrong with it), and have had problems with the 'flush' chain breaking continuosly. We thought we had it set right, and then the toilet starting making a loud moaning noise after flushing. That went off and on for a few months. We haven't heard the noise for a while, but today, a pipe seems to have broken from the toilet. We heard water draining in the basement, and noticed it was coming from where the main floor toilet was placed. We noticed the bowl had completely and spontainously drained, and there was some leaking of water around the base. The pipe seems to have suddenly broken, no one had even used the toilet for a few hours before this happened. What could have happened?

You could have tightened the bowl down to the flange too tight and it eventually cracked. Or the toilet was not actually sitting flat on the floor, but instead, sitting perched on the flange.
