Default Unable to remove control handles/lever

We have a Gerber 3 control shower unit,we wish to remove the old 4 pointed handles and control lever. We have removed the screw in the end of the handles which should then allow them to be pulled off
They will not budge has anyone any advice on how we can remove them without damaging the pipework.
The unit is probably from the 60ís or 70ís .
Thank you in advance for any help.

A handle puller looks sort of like a gear puller...arms with hooks and a threaded rod that pushes the center.
