Do you measure 12 " or 12 1/2" to center from the rough framing for a toilet?

If you are going to put tiles on the wall 12 3/4"

You want 12" from the FINISHED wall, so it entirely depends on what you plan to use there. This does not count a baseboard, unless it is unduly thick or high.
