From the pump - to the controller: bl/wh/ground
from the Pressure switch to the controller: bl/wh/ground

could it be the wiring on the switch. When I put in the new switch I knew that both blacks were on the left, and both whites were on the right, but I wasn't sure of the order (ie. controller black-breaker black, controller white-breaker white)
That is how a 2 wire pump is wired except there is no controller box and the power goes... --> breaker panel, --> pressure switch, --> pump....
For a 3 wire pump the control box is wired in between the switch and the pump with the two hots and ground. Then there is another wire, the start wire, usually red, from the control box to the pump.
So do you have a control box?
If yes, you need a 3 wire pump AND 4 strand wire (colored etc so you can't cross the wires) from it to the pump.
The switch has a two pair of contact points. You connect one of each of the two hots to one pair of contact points, and then the control box or pump wires to each pair of (open terminals) on the contact points. That's so when the points open, both hots are switched off (pump stops) and powered on when the points close (pump runs). You have 4 terminals, 1, 2, 3, 4. 1 and 2 are on one pair of points, 3 and 4 on the other pair. Hots from breakers goes on 1 and 4 (or 2 and 3). Wires to control box or pump go on 2 and 3, (or 1 and 4) the only two open spots.
