Can not identify shower faucet

My shower faucet is leaking. The drain plate says Kohler. Kohler says it is not there faucet because it doesn't say Kohler ON the faucet. Can you help me identify it? The guy at Home Depot wasn't sure. I am not sure if I need to replace the entire cartridge or just the black rubber inserts inside the housing.
