25-28yrs old, sounds like a standard moen valve, if both the screws for the faceplate are located directly below the valve handle it a regular non pressure balance valve. See if there are screwdriver...

Your on the right track. Maybe you will get lucky and the line going into the slab supplies a hose bib if the leak is on the cold line.
Get you some of the thinest foam or neoprene strip material you can find and make you our gasket.

Looking at your pictures don't you need some type of trap primer on the floor drain?? Also, shouldn't that floor drain need a vent?

yes, it will also split the pipe. I see in the pics that pvc male adaptors were used into brass females, that is better than using the pvc females.
I would start by fixing the faucets in the bathroom, may have some debris in the hot side. Additional pressure on the hot side should not be coming out of any faucets, it would pop the relief valve...
I stopped using standard pvc fip adaptors a few years back due to splitting. The ones we use now have a ss band on them and have not had any problems since.

Yes, anytime copper is installed under slab it is type L soft. All of our copper jobs, as few as they are, are piped overhead, we stopped years ago putting any below slab.
Just use plumbers putty in place of the o-ring. The hole size on the lav is standard 1 3/8" opening.
