UltraMaxII with C5 One Piece Flapper Sticking

My Toto flapper valve is sticking. It has the new cage type flapper assembly. Sometimes when it gets flushed, the little white thing falls to one side, then sticks on the rim of the green flapper "cup". The next time it gets flushed, the sticking white thing leaves the chain to short for the flapper to close and the water runs until it you jiggle it. Has any one solved this problem?

In the photo, it seems to show the rod end off to the left. Is that the way it really is, or is it just the way the picture in angled?
