Does a lavatory sanitary Tee / p-trap flange that drains into a combo Wye in the middle of a horizontal drain run with more than 135 deg turns count as a cleanout? Thanks!

That is a loaded question. Are you asking in reference to code or just if it will work?

Cleanouts should be sized to the main line they connect to. Meaning, if that 1.5" line leads into a 3" main line, then the cleanout should be sized @ 3" before it enters the concrete floor.

Is it allowed per code?

The horizontal run in question is 2", so a 1.5" lav inlet satisfies the cleanout size requirement. Does it qualify as a cleanout per code though? Thanks.

Not always, because it is only 1 1/4" or 1 1/2" and a cleanout has to be the same size as the pipe or 4" whichever is smaller. And some codes do not recognize any cleanout that requires that the drain be disassembled to use it. WE always have to install a cleanout tee below the fixture connection tee in the vertical riser if that fixture needs a cleanout.
