utility sink hookup

Hi all,
I would like to hook up a utility sink in my garage/shop. This used to be the laundry room years ago when the previous owner had it, then remodeled. He did numerous renos, and moved the laundry room closer to the house. What he did was, saw off the vent, and the drain, flush with the floor, and removed the toilet. I will have to use a jigsaw, and cut out around the drain and vent, to attach a 3" coupler. Everything is still connected to a 4" drain under the floor, which is ground level. What is the best way to hook up the toilet and utility tub again? Should I:

1: Hook tub with P trap to vent with a "Y" fitting and cap off the old drain?
2: Hook tub with P trap to drain, and then just hook up the vent pipe out thru the roof again?
3: The toilet is easy, cause every thing is there except water line.

I dont know what the 2 holes in front of the old vent stack were for, and I assume the small hole behind the old vent stack was a water line for the toilet.

Can I run water for the toilet from a teed fitting on the wall, when I hook up new taps for the sink?
The tub is not fixed, so i can move it to where I want.

Any and all help is greatly appreciated,
