Would this be doable? I'm assuming I would have to install tile flanges since they probably don't come with this backwards install.

That style of tub does not usually come with tile flanges on any edge. The main problem with it would be "where do you install the faucet", since the faucet ledge is usually on the angled side.

I tore out that type of tub in a diy renovation, from a regular forward facing installation. As noted it had no integral tiling flanges on the back alcove sides, and had been installed without any flange addition or subsitute. There wasn't any evidence of water seepage behind the tiles, but that obviously depended on how much the tub got used by previous owners. They had installed a roman faucet/handshower with holes drilled in the acrylic of tub deck, located at the right angle corner where the curvature provides for some surface area.

I would probably get a drop in tub and install some decent flanges after the fact. This is located on a lower concrete floor. Ive seen plenty of these styles with the faucets on the edge. Besides this is all speculative. I can modify the little back deck portion. I also could if I had to move the walls out in the area where the tub is enough to have 3" of deck around tub.
