Help dismantling Grohe shower valve

This shower valve is about 20 years old. I believe it is a Grohe. I cannot figure out how to dismantle it. It seems as if there should be a removable cap at the end of the handle, but nothing wants to budge. The screws securing the escutcheon plate can be removed, but removal of the plate itself is blocked by the lever. Any suggestions would be appreciated.

This is the closet i could find, but that handle does not look like Grohe and my catalogue goes back farther than 20 years.
