Bathroom Basement Rough In

I am adding a basement bathroom and connecting to the the existing ABS DWV system. I took out the concrete and dug up the lines and attached the new lines for the toilet, drain, sink, and shower drain. I cut the main line that leads outside to the septic system. It was a close cut under the house foundation. The ABS 3" was out of round, and I could not get a coupling on it to attach it to the main system. I placed a ribbed 3" stainless steel no hub coupling on it. Here are my questions:

1. Should I buried this or build an access to the connection? The connection is slightly under the foundation, so I would rather fill in and concrete over.

2. Would you bury that connection? I really wary to bury that connection. The other alternative is to dig up my asphalt parking lot outside the house and try and dig under the foundation and replace the whole pipe section. If I go this option, should I slide the pipe inside a conduit pipe under the foundation. Also, is ABS 3" approved for use under a drive way? Should it had been a special ABS for this? This may have caused the pipe compression.

Any suggestions?

Also, I used a double 3" wye at the bottom section. Is this ok?

I know that I should use 1/4" per foot horizontal runs. Is there a max slope for horizontal runs? I've seen 1/2" as max and some say that there is no max. What is the truth?

That looks more like a back to back fixture fitting than a double combination y-1/8 bend. It will work for you application but many inspectors would reject it. I also wonder why you could not use a glued coupling, since I see nothing in the photos which would lead me to believe you had a space problem. More to the point, however, is what, and how, you are doing the rest of the piping, the parts which are just lying around in the pictures. THAT is where you could, and would normally, create bad plumbing.
 
The pipe connecting to the outside was "out of round", so I could not get a regular coupling on it. The piping lying around were for dry fitting the pieces together.
