Should the toilet flange be flush with the ceramic tile or above it? I am hearing different answers on this.

The flange should be anchored to the finished floor, meaning that it should sit on top of the tile.

How do you drill through the ceramic tile to attach the flange screws without cracking it?

To further clarify, we have a cast iron soil and the flange was removed because we have to extend the flange to receive the new floor. We have a PVC flange extender that fits inside the cast iron and is screwed to the floor. Since we will be putting it over porcelain tile, we can't drill through that to screw it. Do you have any suggestions?

Get a masonary bit to drill through the tile. I'm assuming there is wood subfloor below. If so, get long enough flat head wood screws that will anchor into the the wood below the tile. If the subfloor is not wood, there are anchors you can buy that will anchor into harder surfaces. If the tile should crack slightly around the flange, it's not the end of the world. You can use epoxy or silicone to firm it back up because it will be underneath the base of the toilet and won't show. The main concern is that the toilet flange is stable.

When you lay the tile around where the flange will go, you can notch around where the holes will go so you don't have to drill holes (except through the subflooring). Some tile drills easily, some is a bear unless you use a diamond bit.

Make sure your anchoring screws are either brass or stainless steel. Rust and corrosion will eventually eat away other types of screws.

Before drilling into tile with a masonry bit, chip a small starter spot with an awl or other sharp-pointed object to help keep the bit from skipping and walking until you can get it drilling through the tile.
As said, the flange is secured on top of the finished floor level with only the thickness of the flange above finished floor level.
