Building drain cleanout quandry

I want to add a washing machine drain in the basement, and the current drain looks like this:http://i246.photobucket.com/albums/gg120/86turbodsl/P2250062.jpg
There's a two way cleanout just outside the building foundation at ground level. Do I need another one inside? Can I use a wye or sanitary tee on the vertical to add my washer drain and vent upsteam, or do I have to do something else?
I want to keep it tight to the wall if possible.
and yes I know I need proper supports. It's a work in process.

You would also need to add a vent. Your inlet from the WM might need to be higher than standard to get the required standpipe height.

Yeah, I can get a vent up through the outside wall and join the other vents. I guess the main question was do I need a cleanout inside too?

NO, the only "cleanup" you may have to do is when the sewer stops up and all the sewage overflows the washer standpipe onto your basement floor.

So how does one avoid such a thing then? If it backs up, it's going to back up to the lowest point in the system, it might as well be in the basement rather than in my first floor bath...

actually if your standpipe is lower the manhole on the street and the city main backs up everything would come into your basement, whereas the first floor drain outlets would be above the manhole and wouldn't have this problem. Unless of course you are on a septic system.

In a septic system the only worry about backflow would be if you look out and see a flooded yard with your septic under it.

Yes, by code, ipc, you have more than 135 degrees of change of direction so that bottom 90 needs to be a wye 1/8 combo with a cleanout cap. A more practical solution would be to leave the bottom one the way it is. Then put a comby at the top with a street 90 and a cleanout out the top of the concrete wall. This would allow you to open the cleanout cap outside, eliminating any sewage spillage if the outside line was backed up.
