Usually, 8" apart is normal. This can vary with obstacles, such as studs or floor joists. Pedestal basins are best roughed out of the wall rather than the floor.
Height depends upon the type of fixture. But I usually stub pipes about 8" to a foot above the floor - closet stops are usually about 6" above the floor, while kitchen stubs can be a little higher. When plumbing out of the wall, basin stops are usually just above the waste. There are some unusual fixtures that may require different heights.
