Where do most plumbers place cleanouts in 2 story houses. Do you put a test Tee behind an access panel .

We don't usually install them there, unless there is a horizontal pipe below the floor. A vertical pipe seldom stops up, so the cleanouts are just needed on the lower lever before the pipes turn horizontal.

There will be about 10 feet of horizontal run on a 3 inch line> what would be the best place to put a cleanout and how. in a wall with an access cover?

Yes. A convenient wall, preferrably an exterior one (so any maintenance mess stays outside), with a cover would work nicely.
